RULE 158 ENQUIRY

1) If the Candidate resides OR WORKS in East Lancashire, an enquiry is not required.

2) If a 158 enquiry is required, visit the Provincial website and complete an on-screen form.

3) From the screen, either send the form electronically or, if preferred, a hard copy form can be printed which, when completed, can be posted to the Provincial Office.

4) Or write a letter to the Provincial Office requesting a Rule 158 enquiry. Please include:

a. Name and address (including postcode).
b. Age or date of birth.
c. Occupation.
d. Business address (including postcode).
e. Enquiring Lodge and number.
